Summary: The Clinical Director, Health Centers is a senior clinical leader responsible for providing both clinical and administrative oversight across community health center sites. This role combines direct patient care with strategic leadership to drive high-quality, patient-centered care, operational excellence, and team engagement.. This position supports the CMO and partners closely with operational and clinical leadership to standardize care delivery, improve outcomes, and support a culture of continuous improvement.
Essential Duties and Responsibilities:
Clinical Leadership & Operations
- Partner with health center leaders to align clinical and operational priorities across all sites.
- Conduct regular rounding with Site Leaders and Site Managers to assess needs, identify challenges, and share best practices.
- Foster transparent, bi-directional communication between frontline staff and leadership.
- Promote a culture of team-based, patient-centered care across all locations.
- Lead the implementation and standardization of clinical protocols, policies, and standing orders.
- Drive workflow optimization to improve efficiency, quality, and patient experience.
Quality Improvement & Population Health
- Collaborate with Quality, Population Health and Evidence Based Medicine leadership to improve access, empanelment, and quality metrics.
- Serve as a representative on the Quality Improvement Committee.
- Lead and participate in quality improvement initiatives, including peer review and protocol development.
- Monitor clinical performance and implement improvements based on data and outcomes.
People Leadership
- Provide supervision and leadership to providers and clinical staff.
- Conduct performance evaluations and support professional development.
- Clinical representative for staff engagement, patient and staff satisfaction, and retention.
- Support a collaborative, respectful, and accountable team environment.
Direct Patient Care
- Provide comprehensive, culturally competent primary care within scope of licensure.
- Perform assessments, diagnoses, and treatment planning, including ordering and interpreting diagnostic tests.
- Deliver coordinated and continuous care across the health continuum.
- Collaborate with interdisciplinary teams to support patient education and care management.
- Maintain accurate and timely documentation in the electronic health record.
- Meet productivity expectations while maintaining high standards of care.
- Participate in after-hours coverage and maintain hospital privileges as required.

Qualifications:

Education & Licensure
- MD or DO from an accredited medical school OR NP/PA from an accredited program.
- Active Illinois license required.
- Board certification (for MD/DO) required within two years of hire.
Experience
- Demonstrated clinical leadership experience (e.g., leading teams, projects, or committees).
- Experience in community health, primary care, or population health is strongly preferred.
- Familiarity with quality improvement methodologies and value-based care models preferred.

Pay Transparency: The salary range for this position is $130,000 – $170,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
